Using this convenient service, you can talk to an experienced Memorial Care provider via secure video link and get a diagnosis and treatment plan without leaving your home.Check My HealthThis short questionnaire creates a snapshot of your current health and offers goals and strategies to improve your overall well-being.Patient PortalUse FollowMyHealth or DMH Chart to access your health records, view test and lab results, request prescription renewals, talk to your doctor and more.How It WorksThe first step is to create a profile within the Memorial Care app.
This profile will include your name, birthdate and basic contact info.
Your profile also allows you to add dependents and create emergency contacts.
Your dependents and emergency contacts can also save their spot in line using your Memorial profile.
The app displays all your upcoming and past appointments.Full Integration with Amazon AlexaThe Memorial Care app features full integration with Amazon’s Alexa voice assistant.
Once installed, you can set up a profile and preferred clinic location directly through the app.
